Name Changes
If you have changed your name, you may apply to have this change reflected in your passport. Please note that as of September 26, 2005, U.S. passports will no longer be amended to change items appearing on the data page; you will therefore apply for a new passport in your new name.
If the change is within one year after issuance, please apply at the appropriate Consular office in your district and submit:
• current passport
• completed form DS-5504
• two passport photographs (taken within the last six months);
• documentation of name change (e.g. original or copy of marriage certificate, divorce decree or court order)
• a self-addressed stamped envelope with priority mail (Posta Prioritaria) Italian postage. The envelope should be appropriately sized and with correct postage for the return of the passport/s
Fees: no fee is charged for this service.
If the change is more than one year after issuance, please follow procedure for passport renewal and include documentation of name change (e.g. original or copy of marriage certificate, divorce decree or court order.)
